        of 1930, as amended – New Shipper Initiation Checklist

The following New Shipper Review Initiation Checklist is provided to the public for purposes of further
understanding the new shipper review initiation process. We are placing this checklist on the website,
so that parties may become familiar with the Department’s new shipper regulations and practice dealing
with the initiation of new shipper reviews.

This checklist is intended for initiation purposes only. If the information supplied in the request or
certifications is found to be insufficient or incorrect throughout the proceeding, the Department may
rescind the review or apply adverse facts available, depending upon the facts on the record.
